Join our dynamic team at WCS Healthcare Partners, where you will play a crucial role in supporting individuals residing in scatter-site housing arrangements across Brooklyn, the Bronx, Queens, and Manhattan. Our mission is to promote housing stability and overall well-being through ongoing support, resource coordination, and behavioral health-related assistance.

• Manage a caseload of approximately 25-30 clients throughout New York City.
• Conduct community and field visits to clients in their homes and throughout the community.
• Assist clients with obtaining and maintaining public benefits, including:
  • Medicaid
  • Medicare
  • SNAP and other entitlement programs
• Coordinate behavioral health services and connect clients to appropriate community resources.
• Develop individualized service plans and monitor client progress.
• Support clients with housing retention, crisis intervention, and advocacy.
• Collaborate with landlords, service providers, and internal program staff.
• Document client interactions and maintain accurate case records.
• Participate in team meetings and work collaboratively with fellow Case Managers and program leadership.

• Bachelor's degree preferred.
• Prior case management experience required.
• Experience working with underserved, vulnerable, homeless, or formerly homeless populations strongly preferred.
• Knowledge of entitlement programs, Medicaid, Medicare, and community resources.
• Strong organizational, communication, and advocacy skills.
• Bilingual candidates are highly encouraged to apply.
• Ability to travel throughout the five boroughs as required.
